Health Insurance

Kate's Day Spa has two therapists who partner with client's insurance to cover their treatment. Like other medical services we have adhere to protocol, and are not always accepting new insurance clients. Please call us to see if we are able to accept a new insurance client.

    If we are accepting new insurance clients we will then:
  1. Verify your benefits with your insurance. This must be done on a weekday
  2. Have a current prescription or referral. Your doctor, chiropractor, or naturopath can write this.
  3. Depending on your copay / deductable you may have an amount to pay at the time of treatement.
  4. We will bill your medical insurance directly.
  5. Note: This must be done before your insurance-covered treatment

In order for massage to be covered by health insurance ( in the state of Washington ) it needs to be deemed "medically necessary." A prescription from a doctor or chiropractor provides this proof of medical necessity.

Unfortunately at this time seeking a massage for preventative or maintenance measure, or for the purpose of reducing stress/anxiety is not covered by health insurance.

For treatments covered by insurance there are a few forms to fill out. We recommend you download and print the following forms and bring them with you to your appointment. Please call to verify we are accepting new insurance clients.

We do not recommend you email the forms to us in accordance with HIPAA

Appointment Protocol

Services provided outside of the usual appointment time, including telephone conversations lasting longer than 10 minutes, preparation of documents, preparation for/attendance at legal proceedings, or extensive interactions with insurance companies will be billed at a prorate of $50 per hour. Payment may be made with cash, check, or credit card. There will be a $35 charge for returned checks. Accounts past due over 90 days will be referred to a collection agency. I do not accept pre-payments.

Missed Appointment Policy

When you make an appointment with us, it is time that we reserve exclusively for you. Because of this, please provide us with at least 24 hours notice should you need to cancel or change an appointment. Inability to attend appointments on a regular basis will constitute grounds for termination of treatment. There is no fee for appointments cancelled by our staff.

Cancellation Policy

  • Cancellation 24 hours or more before appointment: No Charge
  • Cancellation less than 24 hours: 50% of treatment cost
  • Cancellation without notice ( no show ): 75% of treatment cost
